We are in network with most major commercial insurance companies. If you have Cigna, Aetna, Blue Cross or United/Optum, please use our book now option to schedule your first appointment. If would like to submit an out-of-network claim for partial reimbursement from your insurance company, we can assist you with this process. However, patients are responsible for full payment at the time of service.
We are in network with both Medicaid and Medicare. Please call our office for available appointments.
In order to see all of our patients in a timely manner, we ask that you please make sure that you are ready and log on at least 10 mins prior to your scheduled appointment. Please arrive to all in-person appointments 15 mins early. If you need to reschedule, please do so at least 24 hours in advance. If you are late or cancel less than 24 hours prior to your appointment, it will be considered a missed appointment and you will be responsible for a cancellation fee.
After a minimum of 3 visits, patients might be eligible for a one-time courtesy refill. However, this is determined by the patient’s individual provider. When sending in medication refills requests, please provide your name, date of birth, name of medication requested, medication dosage, and pharmacy telephone number. If you missed or canceled your last appointment or have been given a courtesy refill within the past 12 months, you will need to schedule an appointment prior to your medication being refilled.
Although it is helpful to have medication records, current medication bottles, there is not a guarantee we will keep you on the same medication as your last provider. Medication decisions regarding continuing or changing medication regimens are always made on an individual basis. We will not enter into an official patient/provider relationship without a minimum of three visits. It is during this review period where we determine if we are able to provide services to meet the needs of the patients.
As experienced providers, we understand how important it is to provide opportunities for us to provide supportive documentation for various reasons. We are willing to write letters for our established patients. Requests for letters must be made in writing and accompanied by a signed authorization of release of information form, along with required administrative fees.
